You are a member of the Settlement Class if, at any time between May 1, 2013 and August 29, 2017, IKS made an automated telephone call to your cellphone, the call featured a prerecorded or artificial voice, and you hadn’t consented to receive that call or you had otherwise revoked any prior consent. If you received one or more such unauthorized calls, then you may visit the settlement website www.HealthCareTCPAsettlement.com to submit a claim for cash benefits.

    This is a pdf form that you must print, file by hand and send to the admin via mail.

    Unable to file claim online as it is not working. No Physical address is given to submit form. How does anyone file a claim for this?

    You print the form, manually fill it out place it in an envelope and send it to the class action admin. Their address is posted above under the sub-heading titled “contact”

    There is no step 4 after certification. This seems like a stalling tactic to me. How can you submit the request with no step 4??????

    You print the form and you mail it. Step 3 is your signature so I don’t think step 4 exists.

    I often receive automated calls from different companies. Is there a registry of numbers that were called? For example, there is a lawsuit against a local cruise provider, they had a link so that you could type in your number and check if they called. My number was not on that registry, but some of my family members were.
    I do not have the capability to go back 3 years on my call log, a way to check if my number was called would be the only way I would know.

    Administrators are not required to place a validation tool. Also in some instances the admin does not know who the defendant called and it is up to you to prove it.

    Had a problem submitting the claim on-line last month receiving an error message for unknown reasons. I retried submitting it today and it went through without any issues.
