For all those that aren’t able to get claim form to open, I recommend trying with a different browser or from phone or other device. It just opened fine for me.

Form wont open for me. Have tried chrome, safari, and iOS. Also, we won the device as a prize and used it and the app. Not sure how to go about getting the receipt.

What if mine is in use and I can’t find the number. To much vibration is going on.

We strongly suggest that you refrain from using the device without taking a break.

  10. Stephanie says:

I bought it at a southern womens show April of 2015 but I don’t know where the proof of purchase would be but I still have everything what should I do

    ADMIN – Hi Stephanie,

If you used the app I am sure the admin has record of your purchase.

  11. Erica says:

How do I prove I’ve downloaded the app and used it since I purchased the product in 2015? Is there a serial number or something like with the product itself? I want to make sure I’m included in both classes and fill out the form correctly. How do I prove the app?

    ADMIN – Hi Erica,

The class action admin will be better able to guide you on this matter. Best of luck!

  12. Gio says:

If I have the purchase on my online bank statement showing I purchased from a store where I live, will that be sufficient? Also, my wife and I both downloaded the app, are we entitled to a claim each?

    ADMIN – Hi Gio,

Contact the admin to check if your statement is sufficient as a proof of purchase. As far as you and your wife having a claim I think every person that downloaded the app needs to file separately.

  17. Heather says:

What admin do I contact for reciept information? I bought mine back in 2015 and no longer have the reciept. I also downloaded the app when I bought it, how would I prove this also?

    ADMIN – Hi Heather,

The appliance has a serial number on the side.

  18. Kristen V says:

I cannot find a serial number on any part of mine. I purchased in June 2016.
I do not have the box.

    ADMIN – Hi Kristen,

The serial number is on the side of the device in little black numbers.

  19. Alisha Young says:

What if I send in a photo of the object? I have no idea how to obtain my proof of purchase. Which sucks because I bought mines in 2011 and it did not own up to its hype.

    ADMIN – Hi Alisha,

A photo might do the trick. Contact the class action admin to see if you can do it that way. Please keep in mind this settlement is not about customer satisfaction, it is about privacy issues.

  20. Ann says:

I filled out the claim form, but it did not ask me for a proof of purchase. Did I miss something? If needed, is the only acceptable form the original receipt?!?

    ADMIN – Hi Ann,

I am not familiar with the digital form for this claim. If you have proof of purchase send it via email to the admin along with your claim #
