3M Dental Crown Debonding Class Action Settlement

Class Eligibility

You are a member of the settlement if you are a dentist or dental practice in the United States, Commonwealth of Puerto Rico, U.S. Virgin Islands, or Guam, who purchased 3M’s Lava Ultimate Restorative and applied the product as a dental crown before June 15, 2015, and at least one of your patients experienced one or more full or partial debonds on or before June 15, 2020. This applies to dentist or dental practices in the U.S., Commonwealth of Puerto Rico, U.S. Virgin Islands, or Guam.

$250 per debond or reimbursement for each out-of-pocket-loss.

Case Summary

3M is accused of making Lava Ultimate Restorative dental blocks that did not function as warranted or represented when the blocks were milled and seated as dental crowns. The plaintiffs claim that the crowns made from Lava Ultimate experienced higher rates of debonding than crowns made from different materials and that characteristics of the Lava Ultimate material caused or added to the debonding.

Deadline

To receive a payment, submit claim form by 08/08/2019 for debonds fixed on or before May 10, 2019. The deadline for the Supplemental Claim Period is between June 15, 2020 and September 15, 2020.
