I am trying to file a claim for the listed product, but the page won’t allow me to put in 1) number of packages, 2) store name, 3) the state purchases were made. Which all are required so I have now tried on 4 seperate times, logging out & back in and other things. Not sure if you have got any other reports about this issue, but I have no way to complete claim to submit. Thank you, Teri Gaulin

Hi Teri, Could you try using a different web browser? Sometimes, data input can fail on one browser (like Chrome) and work on another (like Firefox). If you try another 1 or 2 browsers, and this still fails, then please contact someone at the Settlement Administrator’s office directly. Good luck.
