The final approval hearing was at the beginning of February. I have not yet received reports of checks.

How do I report this on my taxes.

Consult with your CPA. But please note that taxes were already taken from the settlement before it was paid. Your check was net and not gross.
I do not have a CPA. Was this already included on the W 2 from CVS. or are they sending a 1099. This a question for several CVS employees.
