    @Cece Use the registration link first, register, and then you’ll be emailed a Class Member ID to input and go through submitting a claim. Just did it and works great!

      Hi, Sue. Please click on the FAQs link above. From there, you may find the downloadable PDF Claim Form on the right column. You can print it, fill it out, and have it postmarked and sent to them before the Claims Deadline stated above. Good luck.
